Mentoring Program
One-on-one and Group Mentoring Program Information & Applicaton Forms

 

Application Instructions

Thank you for your interest in one-on-one or group mentoring.  To begin the process, you will need to fill out the mentoring program application and TWO release of information forms (see below).  One form provides permission for the State Department of Children and Families (for whom we provide our mentoring services) to ensure that you have no substantiated allegations of child abuse or neglect and the second provides permission for them to review your DMV records, as well as to ensure that you do not have past convictions or pending criminal charges which would impact your ability to mentor.

Because these are legal permission forms, we have to make sure that they are filled out carefully, completely and legibly.  Missing or illegible information means the form will automatically be rejected and will significantly delay your application process. 

 When filling out your forms please make sure that:

  • Every word and number is legible
  • Every line is filled in – either with the appropriate data or with “N/A” if it doesn’t apply to you*
  • True Colors has already input our name and address as well as checked off the box that says “MENTOR”

You can fax the completed forms to True Colors at (860) 232-0049 if you wish. In addition, please mail the completed forms with your original signatures to:
